Learn the signs of a concussion and find Michigan's parent information, acknowledgement forms, and medical clearance form. A player suspected of having a concussion should be removed from play and cannot return without the required medical clearance.

Concerned About Athletics Fees?
Please contact the Athletics Board before deciding your child cannot participate. Tell us what is creating a difficulty, and we will let you know whether any assistance or payment options are currently available.
Need Equipment or Gear?
If purchasing required equipment is a hardship, send the Athletics Board a private note with the sport, item, and size needed. We cannot promise that every request can be filled, but we will review it and respond honestly.

Tell us what your family needs — help with fees, or help with equipment and gear. Requests go only to the Athletic Board and are kept confidential.
